Health Force, LLC is a post acute care management company operating in 24 States across the US.  Health Force has been providing care management services telephonically and in the home since 2013.  Health Force staff performs Health Risk Assessments as well as wellness checks on thousands of people nationwide annually.  Our Care Management services helps to empower the member to maintain their well-being in the comfort of their home environment.  In South Carolina, we have over 50 nurses and social workers who cover the entire State of South Carolina.  In addition to Assessments and Care Management, some of our nurses are trained and have performed data abstraction and medical review projects for other entities.   

 

The management team of Health Force has a long history of advocating for high quality home care services by serving on several Boards and Committees in the Health Care Arena in South Carolina.  Adding to the background and knowledge of the South Carolina community, under the name of Care Pro Health Services, the current owners of Health Force provided Certified Home Health and Hospice services as well as Community based home care services for over 23 years in South Carolina and parts of Georgia and was the single largest provider of personal care services under the Community Long Term Care Program.  In 2010, Care Pro was sold to Addus Health Care.
